With all the specialties and technology, health care today is complicated. We can help coordinate your health care by bridging information between you and your health care team. We will collaborate with your doctors and other providers as needed, to meet your unique needs.

What is Home Health and Home Care?Home Health and Home Care are types of care which are provided in a home setting, but most people are not aware of the differences.
Home health is clinical medical care that is provided by a registered nurse, physical therapist, occupational therapist or other skilled medical professionals. It is often prescribed as part of a care plan following a hospitalization. Home health may be covered by Medicare or other health insurance plans. A doctor’s order is required for home health services.
Home care is non-medical supportive care, provided by direct support professionals (DSPs) or certified nursing assistants (CNAs). Home care support includes such services as help around the house, meal preparation, shopping and errand running, personal care and/or transportation. A doctor’s order is NOT required for home care.
